Case Management
The goal of these programs is to
assist clients in living fully within the community by learning
independent living skills, developing a social support system and
consistently meeting their basic human needs. Case Management provides
goal oriented and individualized support focused on improved
self-sufficiency.
This
service is aimed toward persons who are at risk of hospitalization, or
who require assistance to gain access to needed medical, social,
education and other services essential to meeting their needs. Key
elements are linkage, advocacy and referral. Services may take place in
the individual’s home or at other places within the community.
In
FY2005, 89% of case management was provided outside a Red Rock office.
