General Medical Records Information

  • Medical Records Technicians process requests in the order in which they are received, but we make every effort to accommodate our clients’ needs should there be an urgent request.
  • Requests and releases are processed during regular business hours, 8am-5pm Monday through Thursday and 8am-4pm on Friday.
  • Medical Records Technicians can provide copies of records for services that occur at any Red Rock BHS location. Services performed at other facilities must  be requested from those facilities. 
  • Red Rock BHS has a standard procedure to release records in electronic form, on a password protected thumb drive at zero cost to the client. If you choose to have your records on paper, it will be a cost of $.25 per page up to a $150. Payment will be required for the printed documents before being released.
  • All releases require completion of an authorization form in order to allow for disclosure of your records to yourself or to another entity.

How to Request Records

General Release of Information

If you are a Red Rock BHS client and would like a copy of your records sent to another facility, please print, complete, sign, and submit the following form

Client Access to Records (CATR)

If you are a Red Rock BHS client and would like a copy of your child’s records, please print, complete, sign, and submit the following form.

CATR forms that include an outside facility name will not be processed. Please be aware that this form is subject to an approval process and will make the approximate turnaround time about four weeks.

Typed signatures will not be accepted. All requests must be physically signed.

How to Submit Your Request Form

All requests must come with a copy of an official photo ID (typically a driver’s license).

Denial of your request for access to your records

Under certain limited circumstances, Red Rock BHS may deny an individual’s request for access to all or a portion of the PHI requested under § 164.524. In some of these circumstances, patients have the right to request a review of this decision, and may do so by requesting a review by letter or email:

Physician Requests

All requests for patient records must be accompanied by a signed request form from the patient or guardian, unless your facility has entered into a Business Associate Agreement (BAA) with Red Rock (BHS).
